در موارد شدید، فردی که تمایل جنسی آشکار به پاها دارد، احتمالاً می‌تواند به اختلال فتیشیسم (که با شهوانی کردن اشیاء غیرزنده و اعضای بدن مشخص می‌شود) در صورتی که علائم زیر را رعایت کند، تشخیص داده شود:

تجربه تخیلات، تمایلات یا رفتارهای مکرر تحریک کننده جنسی که شامل استفاده از اشیاء غیر زنده در یک دوره حداقل شش ماهه است.

بحث های نسبی در زمینه فوت فتیش
برای تخمین فراوانی نسبی فتیش‌ها، در سال 2006، محققان دانشگاه بولونیا 381 بحث اینترنتی درباره گروه‌های فتیش را بررسی کردند که حداقل 5000 نفر در آن شرکت کرده بودند. محققان شیوع فتیش های مختلف را بر اساس عناصر زیر تخمین زدند:

(الف) تعداد گروه هاییی که به صورت اختصاصی یک فتیش خاص را بررسی می کردند.

ب) تعداد افراد شرکت کننده در گروه ها؛

ج) تعداد پیام های مبادله شده

نتیجه کار این طور شد که رایج ترین فتیش ها برای اعضای بدن یا برای اشیایی که معمولاً با اعضای بدن مرتبط هستند (به ترتیب 33 و 30 درصد) بود. در میان افرادی که قسمت‌های بدن را ترجیح می‌دهند، پاها و انگشتان پا با بیشترین تعداد مورد قبول واقع شدند، به طوری که 47 درصد از افراد نمونه آن‌ها را ترجیح می‌دهند. از میان افرادی که اشیاء مربوط به اعضای بدن را ترجیح می‌دهند، 32 درصد در گروه‌های مرتبط با کفش (کفش، چکمه و غیره) قرار داشتند.
